How the Dental Forms Work
How the New Patient Form, Oral Cancer Screening Form, Smile Analysis Form, Health History Form, and the Consent Form Work in Minneapolis

Pretty much all the forms our dentist and dental team use in our office are here on-line as PDF files for you see and use.
To understand how the dental forms work, however, one has to know one fact about all on-line PDF files: the information you type into PDF dental forms cannot be saved on your personal computer.
So, to best use the dental forms we have on line, do one of the following:
Either print out the dental form after you open it, then using a pen handwrite your information on the form to bring in to the office when you visit...
Or, enter the information on the computer, then print it (keeping in mind that you can't save what you type to work on later; once you close the form, the information you typed is lost). Filling out forms
Filling out our dental forms in advanced is only for your own convenience, and is certainly not required. If you want to fill out the forms here in our dental office before your appointment, you can (just come about 15 minutes early if possible). - Mai

We also have a very nice, optional dental form from our dentist and dental team that asks new patients about their teeth and smile — a smile analysis — and what things they might want to consider improving one day.
